Condition introduction
In September 2021, Mina was hospitalized in Suzhou due to acute pulmonary edema and dyspnea. She was preliminarily diagnosed as congestive heart failure caused by mitral valve.
On September 25, Mina was referred to our hospital for further diagnosis and treatment. After thoracic X-ray and cardiac ultrasound evaluation, she was diagnosed as stage C of chronic mitral degenerative heart disease (MMVD). There was obvious intracardiac volume overload, and poor circulation led to cardiogenic pulmonary edema.
After careful evaluation ,decided to arrange surgical intervention for Mina in addition to traditional cardiac drug treatment. On September 30, Mina baby performed v-clamp transapical mitral valve clipping in our hospital.
Preoperative X-ray showed that the cardiac contour was significantly enlarged (VHS 11 Vlas 2.6), and there was still a small amount of hydrops.
Preoperative ultrasound evaluation
Mitral valve degenerative disease, thickening and prolapse of valve leaflets, VC up to 7.48 * 3.88mm, suggesting moderate and severe reflux (MR4 +). LA / ao2.44, lvidd36.00mm, lviddn 1.93, left atrial volume 3.34ml/kg and obvious cardiac volume overload were measured by right short axis Sweden method.
surgical program
During the operation, the a2-p2 position with the most severe valve prolapse was clamped and repaired edge to edge. Immediately after the operation, the VC evaluation decreased to 3.70 * 1.87mm, the MR degree decreased by 70%, and the mitral regurgitation was significantly improved.
Conclusion three days after operation
The degree of mitral regurgitation decreased to mild (MR1 +). LA / ao1.31, lvidd32.3mm, lviddn1.74 and left atrial volume 1.15ml/kg were measured by right short axis Sweden method. Cardiac volume overload was significantly improved.
